I had a specific need for this type of bland wall plate cover when I replaced my ceiling fan switch. I looked at other stores and found this one at Home Depot. It worked for me even though the electrical box in place didn't have screw holes that matched the left side of the wall plate cover.

Leviton sincerely regrets your experience. We manufacture wallplates from a variety of plastics. Rigid compression molded wallplates are very sturdy but may crack if the mounting screws are excessively tightened or extra force is applied to them. However, we would like to send you replacements. The screw holes don’t line up on both sides to...
The screw holes don’t line up on both sides to fit a standard box. Look at the “blank” side of the cover. There’s no place on a standard electrical box for those screws. All 4 screws should have standard spacing.
