A:  Are the "old" plaster walls, lath and plaster or gypsum board? If simple screws pulling through gypsum wallboard then over sized plastic anchors inserted may work. If "OLD" lath boards and "OLD" plaster then the holes may be quite large and will have to be refilled, or you can use "Molly Bolts" inserted past the lath for a VERY SECURE mount.

A:  There is a drilling/positioning template provided with the product. Your old toilet paper holder may or may not have the correct spacing, but this shouldn't be a problem since the new holder will have mounting arms that will cover the old holes. No need to patch the wall if new install is holding secure and old holes are hidden.

A:  I'm replacing a broken pivoting bar also and "thought" I had measured it 6 1/2 " center to center for the posts #can't find the same model anymore#, but after reading a bunch of the specs realized it's actually 6.3" like they responded. Check YOUR mount again ..... I suspect you'll find it's also 6.3" since their other common size seems to be around 7 1/2" #but in round metrics#.
