A:  Does it look like anything was pushed into it? I would contact the manufacturer and ask tat question. Along I would see if they would replace it for free.

        A:  No. The Leviton 5641 does that.

        A:  Yes, it is possible to use any 3 way switch as a single pole. All it is doing when switched, is alternating the path of voltage from the black (common, or point) screw between the travelers.

            A:  No, this particular switch is ON/OFF only. There are some model that do offer dimming capabilities. Do some research on Home Depot site.
