A: It covers all 3 and comes with the type B installed in the package. However, after I very, very carefully spread the plastic on my old toliet flush valve assembly (to get the old actuator off)... the blue one fit too sloppy. So I had to go with the green one and use the shaft bolt that is included in the bag but not shown in the picture above (I do not know why they don't show the other parts). * Sadly, if you need to use the shaft bolt, in my bag, was the wrong sized nut. So I had to go to ACE and buy a 6/32 stainless w nylon lock. * I left feedback telling Home Depot of this. It may have been a rare error but I suggest checking it at Home Depot just to make sure if you need to use the shaft bolt like I did. Make sure the nut in the little bag fits that shaft bolt.

A: Most likely a 5. Seek genuine American Standard parts. Chinese replacements are close but not totally to spec and will leak running up your water bill. I have found genuine American Standard on eBay and Amazon.
