Q:If I want to plug this in to just one outlet and have it extend down, is the bottom ground required? Can it be removed or turned flush with the device?

A: I do not believe that it is required, however it would have to be removed to do what you are suggesting and I do not think that you could use the top outlet anyway because the top of this unit would block it.

A: This unit takes up both outlets on a standard wall plate, and is nearly as tall as the standard wall plate. It is about 1" wide when you're facing the wall, and extends out about 1-1/2" - bigger than you'd think. It's very solid, and I like the ones that I bought. But you said it's in a double-wide plate next to a dimmer switch. It will fit into the double outlet, but you'll only be able to plug into the the three outlets facing away from the dimmer. you'll you'll use up two wall outlets to get three, getting only one more usable outlet.
Instead, you might consider a short 3-outlet extension cord. It would only take one of the two outlets in the wall plate, giving you a total of four usable outlets. You'd also get more flexibility on where the outlets are positioned, and it will be less expensive than the product you're asking about.

A: Measuring mine right now it is:
4-1/8 inches tall (so smaller than the outlet plate)
1-3/8 inches wide at its base (nearest the outlet plate)
and it sticks out 1-3/4 inches from the output plate

A: It is ~ 4.25" high and sticks out ~ 2" from the wall centered over a duplex outlet. Appliances plug in from the sides. Ground prongs are up on one side, down on the other. You can butt cabinets against it.
