A: 12H-18D is a diverter stem so it's for the middle handle and diverts water up to the shower head.. The 12H-4D is a hot water stem so it goes on the left side

A: If you remove the pull out spray, does water still come out of the shower head?
If so, feel the water. If it's warm/hot, it could be the hot stem(common) If it's cold, it could be the cold stem. You can do both(with both seats as well) since you have the water off and it's cheap. Diverter is more for if water is leaking out of the tub spout and not going up into the shower head
