A:  It hangs on a picture hanging bracket that comes with the mirror. Pretty heavy so put in a stud.

A:  The frame is brushed nickel. It appears more like brushed stainless, than the warmer look that brushed nickel sometimes has. It's deep on the outer frame sides, but the inside part close to the glass is thin, so the amount of metal showing from the front is small. There is no bevel to the mirror at all, any beveled look is the angle of reflection of the frame in the mirror.
